WHAT TO DO WHEN SOMEONE DIES

Contact your loved one's GP or the healthcare professional responsible for their care to arrange for the death to be verified.

Once the death has been verified, contact White Rose. at any time, day or night. We will arrange to bring your loved one into our care.

The GP will complete the Medical Certificate of Cause of Death (MCCD) and send it to the Medical Examiner.

The Medical Examiner will:

• Review your loved one's medical records.
• Discuss the cause of death with the GP.
• Ensure the MCCD has been completed accurately.
• Contact you if you have any questions or concerns regarding your loved one's care or the cause of death.
• Once the review is complete, the Medical Examiner will send the MCCD to the local Register Office.

Contact the Register Office to confirm they have received the MCCD and arrange an appointment to register the death. They will advise you of any documents you need to bring.

Once your registration appointment has been arranged, please contact us so we can begin planning the funeral.

After the death has been registered, the official Death Certificate will be issued. The paperwork required for the funeral (often referred to as the Green Form or Form 9, depending on local procedures) will either be sent directly to us or provided during the registration process.

Call 999 immediately and request an ambulance and the police.

The emergency services will arrange for your loved one to be taken to a mortuary.

The death will be referred to the Coroner.

The Coroner may order a post-mortem examination or an inquest if further investigation is required.

The Coroner's Office will keep you informed throughout the investigation.

Once the Coroner has completed their enquiries and released your loved one, please contact White Rose. so we can begin arranging the funeral.

If you need advice or support while the Coroner's investigation is ongoing, our team is here to help whenever you need us.

Inform the care or nursing home that White Rose. has been chosen to arrange your loved one's funeral.

The care home staff will arrange for a qualified healthcare professional to verify the death.

Once the death has been verified, the care home will contact us. We are available 24 hours a day and will arrange to bring your loved one into our care.

The care home will notify your loved one's GP, who will complete the Medical Certificate of Cause of Death (MCCD).

The MCCD is sent to the Medical Examiner, who will:

• Review your loved one's medical records.
• Discuss the cause of death with the GP.
• Ensure the MCCD has been completed accurately.
• Contact you if you have any questions or concerns about the cause of death or your loved one's care.

Once the review is complete, the Medical Examiner will send the MCCD to the local Register Office.

The hospital Bereavement Team will guide you through the initial process and ensure the necessary paperwork is completed.

Your loved one will remain in the care of the hospital mortuary until they can be transferred into our care.

The hospital doctor will complete the Medical Certificate of Cause of Death (MCCD).

The MCCD will be reviewed by the Medical Examiner, who will:

• Review your loved one's medical records.
• Discuss the cause of death with the attending doctor.
• Ensure the MCCD has been completed accurately.
• Contact you if you have any questions or concerns about your loved one's care or the recorded cause of death.

Once the review is complete, the Medical Examiner will send the MCCD to the local Register Office.
